We make our patient forms available online so that you can complete them and bring them in. Doing so will save time and help you start your appointment sooner! We are happy to answer any questions about the forms over the phone or when you are in the office.
Your First Dental Exam
At your initial appointment with Dr. Leibowitz, he will take the time to learn about you and your history with dental services. This gives you an opportunity to share what you prefer in oral health care as well as discuss past experiences with treatment that were less than satisfying. The more we learn about you, the more we can provide the best possible treatments in the future!
To learn more about your oral health, Dr. Leibowitz will perform a thorough examination and capture digital x-ray images of your teeth and oral structures. Digital technology provides detailed images promptly while significantly reducing radiation emitted during the photo- capturing process. In addition to evaluating for common threats like gum disease and dental decay, we check for any other potential issues including evidence of oral pathology. After your exam, we will discuss the findings and any recommendations in terms of care.
Your Dental Cleaning
Dr. Leibowitz performs dental cleanings for patients at their appointments. This allows him more time to become familiar with his patients and their dental needs. Specific recommendations will then be discussed so the patient can best maintain their smile at home.
Payment Options
Our practice policy requires payment at the time of service. Dr. Leibowitz accepts most dental insurance plans, but we do recommend that you check to confirm that your coverage is accepted before your visit. We can file claims to your insurance provider on your behalf, and we are happy to help you make sure you gain the most support possible from your insurance coverage.
